Sarah is the ex-wife of Jack Shephard. She was severely injured in a car accident and Jack was dubious that an operation he performed would allow her to avoid paralysis. She does miraculously recover and the two fall in love and get married. It doesn't last, however, and she reveals to him that she's been having an affair before leaving him for good.
